Upon macroscopic examination of sections of the brain, it is difficult to discern the orientation of the fibers. The same is true for conventional imaging modalities. However, recent advancements in magnetic resonance imaging (MRI) make such task possible in a live subject. By sensitizing an otherwise typical MRI sequence to the diffusion of water molecules it is possible to measure their diffusion coefficient in a given direction1. Normally, the axonal membrane and myelin sheaths pose barriers to the movement of water molecules and, thus, they diffuse preferentially along the axon2. Therefore, the direction of white matter bundles can be elucidated by determining the principal diffusivity of water. The three-dimensional representation of the diffusion coefficient can be given by a tensor and its mathematical decomposition provides the direction of the tracts3; this MRI technique is known as diffusion tensor imaging (DTI). By connecting the information acquired with DTI, three-dimensional depictions of white matter fascicles are obtained4. The anatomical images are performed by 3T-MRI, both 3D high resolution T1-weighted images and Diffusion spectrum imaging (DSI ) are acquired. 3D high resolution T1-weighted images enable to reconstruct into various sections, and is helpful in clinical diagnosis. The MRI images belong to a brain tumor patient. In T1-weighted images, a tumor mass is found at deep left frontal white matter. Using DSI-reconstructed tractography, we can evaluate the relationship between the tumor and white matter tracts. The tractography shows three different tracts: corticospinal tract (blue one), corpus callosum (red one), and cingulum (green one). The superimposed neuronal fiber structures are difficult to be resolved by current neuroimaging modalities. Here we demonstrate a novel technology called diffusion spectrum imaging (DSI) to disentangle the complicated fiber structures. The images are acquired on a 3-Tesla MRI machine, including 3D high resolution T1-weighted images and DSI. 3D high resolution T1-weighted images allow us to reformat the images into different orientations, and is helpful in assessment of tumor extension. The MRI images are acquired from a patient with a brain tumor. In T1-weighted images, the tumor contour is ontlined and pseudocolored in yellow. Using DSI-reconstructed tractography, we can evaluate the relationship between the tumor and white matter tracts. The tractography shows three different tracts: corticospinal tract (blue one), corpus callosum (red one), and cingulum (green one). As you can see, the middle part of corpus callosum is totally interrupted by the tumor and the cingulum is pushed into the contralateral side. The corticospinal tract is not effected. The DSI tractography merged with the tumor contour explains the patient's neurological symptoms and helps neurosurgeons in presurgical planning. DSI in brain tumor
